Blank PDF response email attachment

We have an active form, "Test Form - Validated Fields" that is embedded on our website: https://www.sgrxhealth.com/mdhhs-340b-form/. We have been successfully receiving submissions; however today there were problems with the PDF attachment on the two submissions that were received:

Submission: 2/23/2021 at 9:05 am eastern - the PDF was not attached

Submission: 2/23/2021 at 11:32 am eastern - the PDF was blank

A subsequent submission at 2:26 pm eastern today was delivered as expected with the PDF attached.

Are you able to determine why the PDF was not attached at 9:05 am and why it was blank at 11:32 am?
