When I go to integrate my forms to Google Drive, I am not given any options to select. Please help!

Thanks for posting your question. Sorry though but we are unable to reproduce the problem on our end so the problem might be specific with your browser. We'd suggest clearing your browsers cache and cookies and recheck it again. If the problem still persists, try it with a different browser to test.
Tested with different browsers here and it works fine.

I have cleared all history and caches and still showing the same thing..

I was just informed by one of our developer that the problem might be related to an access issue. Since your form belongs to www.jotform.us, can you please check and access the integrations while logged in to this domain?
