I do not know what a Jot form is

Why do I have to do this to get the vaccine. I am 83 years old and I do not understand. I can fill out papers, but I do not understand Jot form.

It seems you have filled a form last Feb 26,2021 regarding covid19 enrollment form.

JotForm is an only form builder and you don't need to create an account if you're not creating a form.

If you have questions regarding the form you have submitted, you may reply to the email you received after submitting the form.
