I'm creating a Grocery Delivery order form. Every item has a checkbox to say the user would like the item and a field for further description (amount, brand, etc.)
For example -
checkbox Tomatoes textbox

You can best achieved this if you use the form's source code (guide: -How-to-get-your-Form-Source-Code)
As you can then move the fields/elements of the form.
Here is a sample page where I used the source code, and just manually move fields in position.
Hope this help. Inform us if further assistance is needed.

I'm new to JotForm and am trying to set up a quick example of what it can do for our users, I appreciate your assistance!
How do I save that Source Code back in to my form within JotForm? I've been searching the User Guide and found something referring to Admin Manager on the left (which I'm not seeing) with the option to Edit Source Code within File Manager (which I'm not seeing).
We would like to be able to edit and update our form in JotForm with this type of format for each item.
Thank you - I very much appreciate your help!

Thank you for contacting JotForm support.
Unfortunately, it is not possible to upload the updated source code back to JotForm editor. For security reason we do not allow upload of any custom script.
Once you download your form's source code and implement the changes, you will have to host the file yourself in your own server.
Hope this answer this question.
