    Is it possible to add a "Middle Initial" field on "Full Name" Quick Tool?

    Hello.  I have a client who has to have a Middle Initial on the Full Name field.  (IE: They need it to be First Name / Middle Initial / Last name in 3 input boxes, instead of the current First Name / Last Name in only 2 input boxes.)

    Is it possible to do this?

    I have created this myself using three unique fields, but I would like to be able to use the "Full Name" field for the form input - so that the users full name is entered into the "from" field.  And, with the custom work-around that I created, i can only choose one of the name fields.

    Hello weborigination,

    You can insert a Middle Name field in between the first name and a last name.  Just click on the full name field, a list of options will show up on the tool bar just above the form, Click Middle Name and select Yes.  You can use that as your middle initial field. 

    You can also click and rename the sub label.
