How to create text box question with multiple rows but no column header?

    I'm trying to create a text box question with multiple text box answer rows, listed in a column but without a column header. I tried to do it using the matrix function but couldn't get it formatted properly.  I want the text listed out as a question, not in the column header.


    Here are 2 examples:


    1. What's your Twitter handle?

    - Official _________

    - Personal ________




    2. What's the link?

    - Eventbrite ________

    - Facebook  ________

    - Google+  ________

    - Other ________


    Any suggestions?

    Using custom css and inject into your form, you can achieve that in your form.

    1. Add textbox field

    2. Use custom css to hide field label

    Here's the CSS: .form-label-left, .form-label-top, .form-label-right { display: none; }

    3. For questions, ex: (What's your Twitter handle?), you need two field in order to achieve that. Text field for label/question and Image Tool for question numbers.
