I have created a jot form and when someone tries to fill it in, it will only accept and email address

I have set up a jotform and it will only accept an email address even in the contact name and store name etc.

How do i fix this??

Kindly please elaborate more about the issue. Do you the form doesn't let someone make a submission when there's an empty field? I've checked your form, and none of the fields are mandatory, so submission will still push through even with empty fields.
