We have had several complaints that forms have been submitted but are not going through. I sat down with a woman today named Lis Martin who filled out an adoption application and she indeed sent it correctly. I have not received it on our end yet. I am very concerned that we are not receiving all apps that are submitted. Since we are an animal rescue, these apps are literally our bread and butter.

Upon inspecting your site, I couldn't see any reason why a submission won't push through. Are you sure that your visitors have successfully sent the application? Were they able to see your thank you message?
It might be possible that the person entered incorrect captcha keywords, submitted the form and then got redirected to another captcha page (this is required), and since she was very sure that she pressed the submit button, did not bother filling out the secondary captcha page.
Information submitted will be lost and not saved unless the visitor enters the correct captcha code.
On another note, I noticed a different issue with your form that is very concerning. The required field validation script for your form doesn't work at all times. In fact, I was able to do a blank submit just a while ago.
This is caused by script conflicts with the form and some other elements on your page. To solve this problem, use iframe codes instead of Embed <> codes for your form.
To get your form's iframe embed codes
1. Go to Setup & Embed tab
2. Click Embed Form
3. Click iFrame
4. Copy the codes provided in the succeeding screen
