I have been issues getting my email notifcations for when a jotform that has been set up was submitted. Everyone else on the distrubution list gets them and then they have been forwarding to me.
I have tried setting up filters but it has not been successful.
Any suggestions are greatly appreaciated!

I've check the form and found multiple issues why the notification is failing to work.
1. All your form notification settings doesn't have sender email on it. I've fixed this for you using the default email@example.com. I would suggest checking this article for more solution on this kind of issue: How To Setup Email Alerts To Prevent NOT Receiving Email Alerts.
2. 12 out of 13 notification email settings are used for our Send Email After Submission conditional logic. But, the form field options used for the conditional logic are not present to the form. I am referring to the Where is School Located field. This field only consists of a single option. Can you please check? OR, are you using the form source codes?
