All I want to do is create a signature to sign a form from the optometrist. How do I do that?

Thank you for reaching out to us. Yes, you can create a form and collect signatures through your form. To create a form with a signature field, please follow the instruction below.

1. Go to "Form Builder" and click "Add Form Elements" on the left hand side of the screen.
2. Scroll down to find "Signature" under "Basic" tab and drag/drop the field on the form.
