How can I view the "Hidden For Your Privacy" items in the confirmation email?

Hello, I am emailing to ask how I can view my appointment date and time made through JotForm? I can't remember what date I chose and I can't view it ANYWHERE. I am inserting a screenshot to show what I mean. Can you help me please?

It seems that you have filled out a HIPAA form and the form owner has marked all fields on the form as PHI (Protected Health Information). Hence you are not able to see the submissions on the email.
