Unable to open forms and fill it

    we have uploaded to the Wix scheduling platform and it was working Great for patients to fill out the consent and submit. Suddenly last week, we only had 1-3 patients complete it online. We thought it was a fluke. We made calls today and realized that patients returning for their 2nd shot were UNABLE to open the link and fill out their form. PLEASE HELP!

    Can you please let us know the URL of the page where the form is located? I was unable to find any JotForms on the provided page. Also, please let us know are there any errors when trying to fill the form? Are your users able to submit the form? Are any errors showing after the form is submitted? Would you mind if we make a test submission directly on your live form?
