How do I make a form mobile friendly (for smart phones)?

    Thanks for contacting us. One thing to consider is to use Jotform's default formatting. Some of the available templates from our form template gallery uses a custom image as the background which is not responsive on mobile devices.

    When using the default formatting, and if there are still issues when accessing form to mobile, we can help you add custom CSS codes to the form that would further tweak the form to be more responsive.

    By the way, I've check your form and indeed, that was a form from our form template gallery. As mentioned, that form uses an image as the background which is not responsive on mobile devices. You may consider creating your own form instead.
