Suspended account: This is not true: form’s purpose is to deceptively obtain sensitive personal information

The information we are collecting is required by the state of NJ for petition purposes. Attached is a screenshot of the petition that was generated by the state.

Please advise what is needed to be done to restore my form as this is critical to my campaign.
