We use the JotForm submissions to send an email to a ticketing system to create a support ticket. We use the "From" address to properly assign the ticket (it assigns the ticket automatically based on this). Previously, I have been able to select an email address from the form to use as the "From" address but I cannot seem to do so anymore (everything comes in with "email@example.com"). Has this functionality been removed? If so, is there a way that I can accomplish this? The "Reply To" address will not properly assign our tickets.

Yes, we removed that functionality by default because a lot of users abused the system causing problems to our servers. You can restore that functionality, but you will have to setup Mandrill SMTP to your account. Please follow this guide: Set Email Field as the Notification Sender Email Address with Mandrill. After you setup Mandrill, you should select "From Address":
