First of all i would like to thank you for making such a great form. This form is even much better than many very expensive or trial forms!
I manage my site with the Wordpress. I have copied this form in HTML editor under footer section on the top row and it work great. But, it appears on the bottom of the EVERY PAGE on the site.
Could you please tell me, where should i copy form, so it would appear only on specified pages

Your form is appearing on the bottom of the every page of the site because you have embeded it into the footer section. So, what you need to do is to create a new separated page for your form.
To embed your form to a single page of your Wordpress site make sure to follow this guide: Adding Forms to Wordpress
