I am getting the following error: There are errors on this page. Please fix them before continuing.
When trying to go past the first page of the form, even tho everything is completed on the first page.
Please Help! Been waiting hours for assistance..

Just tested the form and the first page is working fine now. I checked it on both Chrome and Firefox. We have a problem with text fields for few hours that could be the reason. Can you confirm that the problem is also fixed in your end?

The form works, but does not submit.

I was able to see the non-submission of the multi-page form. It get stuck on the "Please wait..." submit button.
Can you please try re-embedding the form using its iframe code (guide: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code)
There is a possibility of script conflict with the web page where the 2 forms are embedded.
I found out that there is another form (a pop-up form) from here
Using the iframe embed code of the other form http://www.jotform.co/form/33456339880867 should fix the script conflict and allow the form to work properly.
Please inform us if the issue persist even after doing this.

Ok, the form now completes, although with alot of scrolling up... but when it submits half of the results come back empty...

Although I was able to also successfully complete the form submissions now on your website http://newbeginningsmedical.com/medical-history-questionnaire
I confirm you report that there were some missing results on the submission when viewed in the Submissions Page grid.
I found out also the caused of this. It was the Matrix Dynamique widget you used in the form.
I will submit a bug ticket to our higher team support regarding this issue on the Matrix Dynamique widget so that it can be attended to.
For the meantime, the solution I can offer is, replacing the Matrix widget with just the orindary Matrix field found in the Survey tools group in the form builder.
Hope this help. We will notify you as soon as updates are available regarding this Matrix Dynamique widget issue.

Ok, I redid the form using your Infinite List widget in place of the Matrix Dynamique widget.
However now the form will not go to the second page.

Ok, it is paging fine now, however, due to having to put it in an iframe (i believe) I am having an issue were not all elements are shown. This has occured in two locations: page 3 of form, if you select YES to PSA question and try to input date via calander, the calendar gets cut off, and then page 4 if you select YES to all questions then half of page 5 gets cut off including pager buttons at bottom.
