Can I Use a Form on Multiple Sites?

Yes, you can use a single form on multiple sites, as many as you want to full fill your needs and requirements. While your form is used on multiple sites, your form submissions from multiple sites will be viewed on the same page. 

All you have to do is to create your form and embed it to your sites using one of the Embed Methods described in the JotForm User Guide.
