I developed a Contact form using a template that I modified. I saved the form, copied the HTML to embed it in my website, and added it to my website. I didn't like the look (the submit button at the bottom was cut off) so I went back into Weebly and my form was gone! The only Contact Form left in Weebly was the original unmodified template.

Do you mean that the form disappeared from your JotForm account or your Weebly web page? Please also provide us with the link to the form if possible.

It disappeared from my Jotform account, but never mind. I deleted the template Contact form and made a new one from scratch.

I just checked your account and can see your forms
I also found some deleted Contact forms in the trash folder -- you can restore the form there in case it was not for deletion.
In any case, please do not hesitate to inform us if there are issues.
