    Hi there,

    I took a look at the last form you worked on and see that the Thank you page is set to

    http://www.trinitystreats.com/pages/ThankYou.php

    Do you have any access to that ThankYou.php file?  The 404 message is legitimate because I cannot access that file using a browser.  It looks like it's there because the webserver isn't redirecting- it's just writing out the 404 file for any bad url  for example,  

    http://www.trinitystreats.com/pages/blablabla.php

    will produce the same 404.  Watch out for CaSe SenSITIviTY, perhaps the file is thankyou.php rather than ThankYou.php?
