Dear jotform team and members,
I am trying to include a check box in my form, but I only need one option (it is to accept the terms and conditions, so it is mandatory to click the box to send the information). However, in my website there are always 3 options. I have tried to delete two of them but after some time it add two more boxes. How can I solve this problem? Maybe a check box is not appropriated in this case?

Here's how you can create your Terms and Condition Settings: Adding a License Agreement to a Form. Please kindly re-check your settings and make sure to do the same. Also, don't forget to save the form by clicking the "save" button from the form builder.
