How can I create a specialized form?

    I have a PDF with images, measurements, etc. and the information that needs to be filled in needs to reference those particular image locations (see my most recent JotForm).

    Can I create a form that matches the image/form positions?

    Can JotForm host the PDF so that users fill the PDF locations?

    What's my best option for something like this?

    - Can I create a form that matches the image/form positions?

    As far as I can check, the web form fields are perfectly mapped to the correct PDF positions.

    But I believe you want to add the exact diagram images to your web forms and place the the textboxes on these diagrams at respective positions as well. Is that correct?

    If yes, then unfortunately there is no direct way to achieve this but there are some workarounds.

    1. One workaround is to create different bigger images for the diagrams of Hinges, Accessories, Corrugated walls, Front view of trailer etc. In these diagram images, give names to the fields like "Hinge 1", "Hinge 2" etc. (or something similar); so that users can see the diagram and the field label associated with the position.

    2. Then, right below each diagram, place the fields with the exact labels you gave in the diagram; so that user knows which field he/she is filling. Repeat this for all of your diagrams/fields.

    3. Then, in the end; activate 'Preview PDF' button for your form as well; this 'Preview' button is placed right next to the Submit button; I noticed that this is already there. Users can view their values and positions on the PDF itself before submitting the form. You can add a note regarding this using the paragraph element right above the Submit button, informing your users about this; telling them that they can first check the values in the PDF before submitting it.

    - Can JotForm host the PDF so that users fill the PDF locations?

    The PDF that you uploaded in your Smart PDF form is already hosted on JotForm; the filled PDFs will be available for you to download from under the PDF Editor, Inbox as well as Submissions section of your form. You can download this filled PDF anytime you want.

    Hi Patrick,

    Given the complexity of the form, I'm more interested in having people fill out either the PDF and JotForm captures the data or importing the images and overlaying the boxes where applicable.

    Is this a possibility?

    Please note that when you import your PDF into JotForm, users cannot directly fill on the PDF.

    But users will need to fill out the online form fields on JotForm (which are mapped onto your original PDF). Only the submission data can be shown on the original PDF.

    But, please note that at the moment, file uploads are not supported on Smart PDF forms i.e. images uploaded on online form fields are currently not possible to show on the original PDF.
