I worked all day on my contact form. I read the forum. I believe I've done everything correctly to add the form to my website, but it comes up blank. The contact form has its own page, a test button link on my master page, but nothing!
Can you please help. thanks

I think the problem is the DOCTYPE declaration
To resolve this, please replace it with:
<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">
Otherwise, please use the iFrame version of the form
