An account I created was suspended

    I created an account under the email address for a client under the username McCartneyProduce. After the forms were live the account was suspended for phishing. It was employment applications and Vendor applications. The client needs the form live for his website. Can you tell me why they were suspended?

    As checked, your account has been suspended because one of your form contained a Copyrighted Logo and SSN field. It is considered as a scam and violates our Terms of Use.

    We understand that this might not be the case with your forms, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately. 

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to with Thread #3150838 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just remove the logos and lift the suspension of your account.

    Also, forms asking for Social Security Number needs to add the following widgets for compliance.

    1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

    2. Add a Signature widget where your users sign and agree to provide such sensitive information.
