Ident.com sending emails to our patients after they submit a form

Have you all seen this occurring? This company is sending emails to our patients on our behalf after they submit their paperwork via JotForm.

As shown in your screenshot, I can see that the email originates from an email address associated with the domain ident.com. Please reach out to them to seek clarification.
