How to create a numbered, repeating list?

Good morning,

I am looking to create a form that can document observations from a site inspection. The ideal format would be a numbered list. Is it possible to created an automated numbered list where the person filling out the form can list observations and have them show up as a numbered list? Is that available via a widget? Or perhaps there is a text box available that allows formatting with numbering? I would appreciate your insight.
