multiple hidden conditional elements still showing up on PDF

Please advise. We've spent a lot of time trying to set up the conditional settings for our Treatment report card so that "unchecked" options do not show up on the PDF, however there are many headers for short text answers that are lingering regardless of their hidden status. How can we navigate this?

We are happy to help you with the PDF and conditional settings.

I cloned and tested your form, but on the clone, the questions seemed to be hidden correctly.
When I submitted the cloned form as empty, this is how I received the PDF:



Please note that the question titles will show in PDF Editor when you are editing the PDF layout.
Once you submit the form, the created PDF report (which is also included in the notification email), should not include the hidden questions.

May I ask if there is a certain pattern, e.g. choosing specific questions, when the hidden question titles show on the PDF?

Would you also allow us to make a test submission on your form, so that we can test the issue further?
