Create a material picker form

Additionally, we are trying to create a material picker form but cant find a suitable template or the right widget to use. Basically, as a part of our form, we want our staff to collect information on what materials they need for their job so we can order them. The need to provide the description of the part (from a drop down) and the quantity they require. The description needs to show the part number and product description in two separate lines. I have included a sample of what we need to create in the screen shot attached. Have you any suggestions? we have tried, spinners, product lists but can't seem to find what we need so can you help?

You might consider using Input Table instead.
