Can the information on the medical papers / forms be proved as reliable ?

    Please note that JotForm is an online form builder that lets users create online forms.

    I am not sure what exactly you are referring to.

    I'm quite not sure if I got your query correctly, but data can be verified thru an email address.

    Could you clarify which medical paper are you referring to?

    Have you filled out a form that was created on JotForm?

    Do you have a form link that we can refer to?

    I'm asking is whatever info that your clients put on your forms is

    verifiable including these form types uploaded by your firm ?

    Patient medical record

    Professional medical history

    Personal medical history

    Past medical history form

    Do you put the info on your patients' forms to the hospitals / clinics that

    are carrying your clients' medical papers in their databases to be sure

    that the info is verifiable ?

    Please note that we do not create forms, but we are a platform for users like you to create forms. Also we, JotForm have not uploaded any form types as you have mentioned.

    Are you referring to the form templates that we have here: https://www.jotform.com/hipaa/templates/?

    If yes these are sample templates that you can use on your JotForm account to collect information from your customers/clients.

    Thanks for the clarification.

    Those are blank form templates that you can use in your account.

    The data entered via those forms will be stored in your JotForm account on our servers/database.

    Currently the option to host data on separate servers is available only under our JotForm Enterprise plan: https://www.jotform.com/enterprise/
