I created a survey and have it set up to send me e-mails with submission.
I used the source embed code on my website. I copied it exactly and it shows up correctly.
When I test the form, it will let me enter information, but when I press submit it does not take my to the confirmation page.
I did not receive any type of e-mail for the submission and there is no change in the number of submissions in my statistics.
Is there something I need to change? Did I do something incorrectly?

You have embedded the form's source codes inside another form that is why submissions are not collected by our server.
Our servers are not receiving your submissions because the form follows the action url in this other form instead of "https://www.jotform.com/submit.php" as defined in our forms' source codes.
To solve this problem, delete that other form or place your Jotform outside of it.
