Upon checking your tables here https://www.jotform.com/tables/211872359503154, it seems that you deleted the old appointment field and now it's showing as blank on the tables.
Can you please try submitting the form again with the new appointment field settings and let us know if it still allows you to submit more than 3 person per time slot?
