First of all, let me inform you that your form is hosted in a guest account, non-registered. That put in risk your access to your form and its received data. So, please make sure to create a new account by going to this URL: http://www.jotform.com/signup
Once you create your account contact us in a separated thread and request us to move your form to the new registered account and share with us the URL of the form and the Username of the new account.
