I need help to make form

I need a form created actually as it appears on the pdf. file. Is that something the support team can help me with?

We will be glad to help you with your concern. Do you require the online form to be in the same style/format as your PDF Form? Or, as long as the online form has all the fields that the original PDF Form has?

I don’t require to look completely the same but it would be great to get
the information correct.
Here is a copy of the form.
I have the original version and then the completed version. I ultimalety
will be trying to convert the form submissions into the completed version.
