    Hi,

I would like to include an image in my questionnaire for the person completing to refer to.

    something like:

‘in the graph below, you can see 5 outcomes at the end of year 1’

    INSERT PICTURE Of GRAPH

    which would you choose?

then a multiple choice question for outcomes 1-5.

this seems simple enough to do on Microsoft forms and Typeform, but really can’t work it out in the Jotform platform.

    your help would be appreciated.

    Hi,

You can set it up like so:

Add a Paragraph element for your image and caption, and a Single Choice/Multiple Choice/Dropdown element for the choices.

Please review and see if this setup could work for you.

thank you Ariel for this, I have found that this does assist for the description and image.

Is there a way to ensure the question/Answer section is also on the same page, rather than on the next page?

below is an example of it done on Typeform, which I wish to replicate something similar here.

    Hi again,

Checking your account, I can see that you're using the card form layout which displays one field per page.

So, I recommend switching to the classic form layout which allows multiple fields per page.

    Thank you Amin.

The above solution indicates that having an image in the question description along with the multiple choice question/answer cannot be achieved on one page via the card view.

The card feel is preferred given it keeps focus on each question. If via cards they need to move to the following page to select an answer, it will have the user flicking between screens to remember what the question was. Not a great user feel.

My preference is to not do the one page layout as it looks like one, bid daunting questionnaire to complete.

Can you advise if having a description + image + question on one page in cards (similar to the Typeform example above which is using card layout) is something you will be able to offer anytime soon?

Sorry to keep coming back to this but want to know that the solution we choose can offer what is needed before committing to a paid subscription, with some of the above standard with other form solutions.

    thanks for your attention

    Hi again,

Kindly note that you can still have almost the same flow as in Card Forms using a Multi-page form with the Breadcrumbs widget in Classic forms.
