Can you please clarify how you used the form inside the e-mail?
You cannot really embed a form inside an e-mail message.. the most you can do is add the link of the direct URL of the form within the e-mail message like this
If you can open this message in your iphone, you can test the link of the form above.
Test submit the form within your iphone browser.
Hope this help. Please inform us if issue persist.

<FORM id=40173349218453 class=jotform-form accept-charset=utf-8 method=post name=form_40173349218453 action=http://submit.jotform.me/submit/40173349218453/><INPUT type=hidden value=40173349218453 name=formID>
Here is the problem,
You need to correct the source code.. name=formID> Please replace the formID with the actual formID while generating source code.

Thank you for your response.
The source code works as it is expected, please see a demo here
Please note that most email cliets will not allow any scripts to run in an email message, which is why why the source code will work on a plain HTML file but not on the email body.
The best way is to share the link to the form, rather than the form itself.
Chances are, even the emails you send out with the embedded form will be marked as spam, and you could be blacklisted by some services.
This method is perfect for same-domain email's who expect that kind of content in their emails.
