Uploading mp4 files to form

Why can't mp4 extensions be uploaded onto forms ?

If you mean uploading them using the File Upload widget, they can - the extensions present on the list by default are just the most common extensions.

You can simply add mp4 to that list (or remove the list altogether and add mp4 to it if you only want your users to be able to upload mp4 files) to allow the upload of mp4 files:
