Can't log-in to the site.

Hello support team. I can't log-in to the site, and when I try the Forgot Password, the system is not sending me a re-set email. Please help.

The email address you've given us is not associated with any account. This is why you are not receiving the password reset email. Kindly check the email address that was used when you created the account.

If you have a form URL then we can also use it to find your account. Looking forward to hearing from you.

This is strange. I'm 95% sure I used the mike@sequentplanning.com email to set-up jotform. I checked other possible emails and don't see any password reset emails.
The URL that we placed the form is unitedwealthadvisors.net. We are trying to update the calendar for available times.

I am having the same problem, but after I reset my password it says "Account Locked. Your account has been locked due to 10 failed login attempts. To keep your data secure, reset your password.
