Formatação Tabela

Boa tarde,

Eu estou a ter algumas dificuldades em formatar uma questão do meu survey.

Por exemplo eu gostava de ter a seguinte tabela com o campo "Target Value" previamente preenchido por mim. Como se fizesse parte da pergunta (como a coluna n1).

Existe alguma forma de ultrapassar esta questão?
