Every time a person submits this particular form, the images are broken in the PDF sent via email. The only way I can get the correct information is going to this form inbox and pulling the information one by one.
Could you please see what is wrong with this form?

I was able to replicate the issue.
Can you please try deleting the old PDF document and create a new one? Kindly try that and see if that works.

I tried this and I am still getting an error. Any other ideas? Or do I need to completely redo the connected form and this one?

So, I had the idea of creating a whole new form, creating a connected pdf, and redoing my conditions. That seemed to have fixed the problem.
