So I can pass e.g. Name, city etc to the form and this data are shown up in certain fields of the form. Please use this app to do so: http://apps.jotform.com/app/import_data
Your data needs to be in a spreadsheet or in CSV format.
Follow the instructions precisely in order to match the fields of your data to the form.
