The sub label text is a very useful option, and is available in the drop down menus and most other types of data entry field, but not for some reason in the check boxes and radio buttons.
I am designing a form that will use lots of check boxes and radio buttons, and I would love it if sub label text were available in these.

Sub-labels are commonly seen below the field's input box, such as in these examples:
The reason why check box fields and radio buttons don't have sub-labels is because there is virtually no limit to the number of options that can be added. As such, adding and managing sub-labels under the options can prove to be cumbersome and messy. It would also make the form look too busy and chaotic.
Anyway, I will still submit a request ticket to see what our developers can do, although I cannot guarantee anything, especially now that our developers are busier than ever.
In the mean time, you may download the form's source codes and then implement what you had in mind by customizing the form's html codes. You can also edit the form and add anything you want using a web editor such as Dreamweaver.
To get your form's source codes:
1. Go to Setup & Embed tab
2. Click Embed Form
3. Click Source
4. Copy the codes provided in the succeeding screen or download the whole optimized codes
