I go to My Forms, and it shows my form Event Scheduling. But when I go to edit it, I don't see the whole form, only three fields. The form still displays correctly on our website. I haven't edited it in awhile so I may be missing something I have to do to see the whole form.

If the form is not being displayed fully in your browser, can please try refreshing the browser while still having the form open in your form builder for editing.
Use the Refresh button of the browser, or press keyboard key combination CTRL+F5 -- this will refresh the browser.
Inform us if issue persist.

I noticed that the embedded form in the website http://uusalem.org/Event-Scheduling-Request belongs to a different JotForm user. Are you also the owner of the account that owns this form http://www.jotform.us/form/22285495575162 ?
If you are, you must login to that account to be able to view the form in form builder.
Yes that is the form. I'm puzzled. I logged in with the information I had written down. It's been awhile since I edited that form, but I did edit it and I did get the html to put it on our website. One of my email addresses is one of the ones that the form goes to, either email@example.com or firstname.lastname@example.org. How do I find out how to log in as the other user? I'm confused.

Ah, I found the other login information. I didn't realize I had two accounts. Thanks for the help.
