Our online waivers are not coming through. The last email that came through for one of our waivers was on january 28th. can you please contact us ASAP. The utilization of these forms are vital to our business.

Please note of the reason and to avoid this from happening again, we would suggest reviewing this guide: How To Setup Email Alerts To Prevent NOT Receiving Email Alerts & Bouncing Issues and follow our recommendations such as whitelisting our IP Address and Domains to your email hosting provider.
Also note that your Reply-to and Recipient settings should be completed with Sender Name, Sender Email and the recipient. I checked some of the forms, and they turns out to be not well configured. The Sender Name is missing. Without the correct settings, this would affect the functionality of the notification email so with the autoresponder email. Kindly fix it as well.
