Hi, that form is hosted in that domain (jotform.me). What you could do, is loggin to jotform.com, and clone that form while logged in that domain. Then you could embed the form in your site. Let us know if that solves the problem.

Thanks David for the reply but now there are three sites I am in.
I signed up and created my Jotform and use the Link at JOTFORM.US
the form the invalid form URL took me was JOTFORM.ME
and you are suggesting me to clone JOTFORM.COM
I added the link http://form.jotform.us/form/40306791903453
and I cant change the mentioned link of my form, I like to keep the jotform.us link, can that be fixed so it works well for my site.
I appreciate your support

I am not sure how to specifically use a particular domain. We have a domain separation rule and every form as its own assigned domain name. The assigning of the domain name is automatic and based on your geographic location.
JotForm.us domain is for USA user and JotForm.me is for asian user. There is no possible way to choose which domain you want to assign to your form as this is done automatically.
Do get back to us if you have any questions.
