We are still having trouble with submissions. I have set the word limit at 500 for our form and submissions with far less words are truncating when a < or > is used.
I have inlcuded the text that was submitted as well as a screen shot of what came through the submission. This has now happened multiple times. Can you help?
Title: Pharmacokinetics of Loop Diuretics Administered to Children per Standard of Care
Furosemide is a commonly used loop diuretic in the treatment of edema and hypertension in both acute and chronic disease states. Despite its widespread use, we have little pharmacokinetic (PK) data in children. Dosing is based largely on PK data from studies involving healthy adults. Diuretic responses may be altered secondary to age-related differences in renal function or by disease states.
The purpose of this study is to evaluate the relationship between serum creatinine and clinical indication on furosemide dose related to urine output.
This study involves both retrospective and prospective components. A retrospective chart review will be done for patients previously on oral or intravenous furosemide and data such as postnatal and gestational age, weight, furosemide dosing history (actual dose given, route of administration, start and stop date), net fluid (gain or loss), serum creatinine and clinical indication will be collected. Data will be collected for patients less than one year of age who received at least one dose of furosemide.
The prospective part of the study will involve patients less than one year of age who are receiving either oral or intravenous furosemide as part of their routine clinical care and are likely to survive beyond forty-eight hours after enrollment. Exclusion criteria will include infants weighing <500 grams or too fragile to tolerate blood draws, legal guardian refusal to sign informed consent, significant renal dysfunction or anemia. At the start of therapy, the most recent plasma creatinine, birth weight and length, gestational age, birth history, race, gender, day of life, concomitant medications, current weight and length, urinalysis, start date of loop diuretic therapy, feeding method and nutrition type will be collected. Blood, urine and DNA samples will be collected.

This is a known bug in the TextArea field in plain text mode when the value contains less than/great than sign and immediately followed by text. The succeeding texts/contents are trimmed after submission.
To get around this, you will have to set the TextArea field mode to use Rich Text content.
I will submit a bug ticket to our higher team support so that it can be check as soon as possible.
For the meantime you can try enable the Rich Text mode of Text Area field for the sign/symbols to work.. I tested on this mode, and so far the field did not parsed and trimmed the content.

This issue confirmed and resolved. Please let us know if it is persistent.

the form through April of this year.
We will probably not need this form again until 2015 and I imagine there
will be a new version of the form and we will not have an issue with this
