For some reason our integration had stopped. Yesterday I was able to integrate the jotform to Salesforce again. Yesterday I did a test and it worked. However today it doesn't seem to be working.
I have noticed that our configured 'Thank you' custom URL, shows a message rather than the URL provided and configured? This is the message: “ Thank you for wanting to become a volunteer with Miracle Babies, please register using this form. If you require assistance to complete the form please call us on 1300 733 664.” http://www.miraclebabies.org.au/get-involved/volunteer/volunteer-registration/ . It doesn’t seem as though it submits the form?!
I checked the ‘Thank you’ URL by Copying it and placing it into an internet window and it displayed the Thank you page: http://www.miraclebabies.org.au/get-involved/volunteer/volunteer-registration/thankyou/. I have since done 9 tests and they have not displayed our‘Thankyou’ URL at the end.
I don’t receive an email notification either and it doesn’t stream into Salesforce, I originally assumed that the form is not being submitted or finalized, however when I went to 'My forms' and looked up the submissions they were there. I also checked that the notification email is being sent to my email and it seems to be correct: email@example.com
I additionally have noticed that the CRM integration does say that it is connected, but with no recieving the email notification, I am not also seeing the submissions in Salesforce either. Also there seems to be a ghost field which I didn't create when mapping. which is very strange!
Any suggestions on this would be appreciated.

In my initial observation with the custom URL Thank You page of this form http://www.jotform.co/form/32281432192852
The custom TY page is not being displayed properly because the URL address is not complete.
Make sure that the URL is the exact path address and it includes the extensions(.php,.html, .htm .aspx... so on)
Please try completing the URL and test again.
I also suggest you refresh the integration of form to Salesforce and check if this resolves the submissions integration failure. We currently do not have any reported issue involving Salesforce integration, so, this most like an isolated case on this form.
I am not able to understand what is the 'ghost field' you mentioned about in the field mapping process. I suggest you also include screenshot image about this next time you reply.

Thanks for your response, however I don't understand why it needs those extension and plus I don't have a clue how to do create it with the extension.
I just copied and pasted the URL for our Thank you page into a new internet browser window and it worked?
Please find attached the URL of the 'ghost field' that I mentioned above. I can't delete this field.
I have also just tried to delete each field until I got to that particular one and it still won't delete.
I have tested the stand alone of your form and it does not appear to behave properly. Yet I have created a clone of your form and not only do I get the email notification. I am also redirected to the thank you page.
Kindly clone my clone of your form back into your account, and try it out. Also by creating the Salesforce into it. Do let us know if the issue persists.

I tried to do as you suggested and follow the instructions and it didn't work.
I am confused and don't know how to create HTML based forms. I know that you created a clone but when I followed the instructions that you have above, it got me into all sorts of a mix.

I am not very IT (or Jotform) savy however as you can imagine this form is important to our organisations processes.
Just copy the link provided by my colleague Cesar then paste it to the Import Form URL field.
Here's how you can clone the form:
1. Copy the URL
2. Click on Create Form
3. Click on "Import" button
4. Paste the URL that copied from Cesars cloned form
Once you have successfully cloned the form you just need to re-integrate salesforce and test your form once done.
