Apparently, there is an issue when a ceratin name is selected from a drop down list, it shows all of the following fields rather than just for the selcted name. In the conditions, I set up to show only one field and hide the rest when a ceratin name is selected.
Can you please address this?

You do not have to necessarily add hide conditions if you are showing that fields based on some condition. If the condition does not match that field will be hidden anyway.
