Form intergration with Shopping Cart?

    Hello, I basically have a website built through wordpress (Themeforest) and I am looking for a form that can be integrated into my existing website that can communicate with a Shopping Cart so my customers can fill out my Service Order Form with all their service criteria and then add that form into a Shopping Cart, (Add to cart button). Once at the shopping cart, give my customers the option to continue shopping (Continue shopping button), so they can come back and fill out as many Service Order Forms (With the JotForm form) as they want and be able to pay for all the calculated service order forms at once (Proceed to checkout button), instead of separately.

    I have attached an example of what criteria my Service Order Forms needs to look like and the functionality it needs to have (Calculate the amount chosen by the customers on the form). The first form is a Members form and the second form is a Non-members form which has two different SERVICE prices embedded into each form based off a customers member or non-membership status.

    Is this something that your form service will be able to do with the form built through Jot Form and then be able communicate with a shopping the way I described in the 1st paragraph. If not, is there another  way to accomplish this or is there another form builder service that provides this service all in one? Please, advise me at your earliest convenience if this can be accomplished with your form service, thanks!
