I created a Time Off Request form. I would like to be able to have employee submit this for to his supervisor, when the supervisor receives it, it must check an approval box and submit to his supervisor for final approval. If the secon supervisor checks approval, then the form returns to original sender. Is this at all possible?

Yes, this is quite possible.
You would need to have at least one email notification set up for it to work.
Then, create conditional rules that will send an email based on an action on the form, like clicking a checkbox approval - This linked guide will shed some light on how you can implement this.
