I have a dropbox account I used to login to create this form: http://form.jotform.com/form/11774838352
I'm having trouble logging in to our jotform account. Here is our dropbox account info:
Please reply back with access to our account.

I couldn't look up a Jotform account using the email addresses email@example.com or firstname.lastname@example.org. Can you provide us your Jotform username? If you can't remember using a username to log in to Jotform, it could have been that the account you were using was an unregistered or 'Guest' account.
Also, the form you have provided is actually deleted - http://www.jotform.com/form/11774838352
http://form.jotform.com/form/11774838352 is actually an un-updated (undeleted) version of that form, but the reality is that it is already deleted.
I suggest that you register for an account so that you will not lose access to it. You will also need to re-create your form, since it is already deleted.

I am not sure what happened but I have recovered this form, created an account for you and assigned this form to it. The new URL:

Your new form appears to have been integrated to dropbox. I am not sure if the integration was inherited from the original form or if you redid the integration yourself.
Whichever is the case, it is still good to know that your form was restored. Feel free to search or ask around the forum if you have other questions in mind.
